Booking Form

If you are new to the Sunflower Effect courses and haven’t yet told me much about yourself, please give me as much detail as possible. These notes will enable us support you better through the course you’ve booked, as well as comply with GDPR. Anything you tell us here is strictly confidential. Please note you need to fill in the boxes with a * or the form will not submit.

  • The place where you are living whilst doing the course. It is very unusual that we would contact you by post except if we are unable to contact you by other means.
  • We use this to connect you up with other group members for the purposes of supporting one another – most participants prefer to use a personal email. This is optional. If you are booking on behalf of someone else please tell us their email.
    Be aware we offer the course at discounted rates if you are paying for yourself. Please let us know if the situation changes and your employer offers to sponsor you.
  • i.e. by instalments; electronic transfer, cheque etc. (If you have already made payment please tell us the date of transaction)
    It’s really useful for us to know how you heard about the course. If you heard about it through a number of sources, please tick more than one box
  • Please tell us if you need any more information related to your booking, have a question etc. If you are booking for someone else, please let us know any special instructions. It is really good to have both of your email addresses and contact numbers for our records.
